Appointment
Before my cardiologist consultation
Please bring the following items to your appointment:
- Doctor’s referral
- Current Medicare card
- Private health insurance fund name and member number
- Age pension or Centrelink health care card (if applicable)
- DVA card (if applicable)
- Any recent test results (if available)
- A list of all medications with dosage and times. Alternatively, bring them in their original packaging.

New Patient Registration
To help your appointment run smoothly, you can complete your registration forms before you arrive.
- Download the editable PDF forms below.
- Type your details directly into the forms and save them.
- Email the completed forms to your clinic before your appointment. (Obtain email address from your relevant clinic here)
- On the day, please still arrive 10–15 minutes early to finalise your check-in.
