Improving Early Detection of Heart Disease

HeartPath

A partner-supported program providing eligible patients with access to bulk-billed specialist cardiac assessment, regardless of location or circumstance.

Our HeartPath program is a structured pathway designed to support timely access to specialist cardiology assessment to identify heart disease earlier, with no out-of-pocket costs* for eligible patients.

  • GP, Specialist or Nurse Practitioner referral required
  • Bulk-billed cardiology assessment for initial and follow-up online consult and testing*
  • National access to specialist cardiac care

Family history, cholesterol, obesity, high blood pressure and lifestyle can all increase risk – even without symptoms. Many Australians delay follow-up because of cost, access, or uncertainty about what comes next.

Our HeartPath program removes these common barriers by providing a clear streamlined pathway, specialist access no matter where an individual is located, and no out-of-pocket costs for eligible patients*.

*Applies to tests with MBS item codes. If further management is required after initial consult and testing, options will be discussed with the patient.

Who this program is for?

This program is designed for patients referred through suitable partner programs. Considerations for referral from our partner programs may include:

  • Patients considered to be at elevated risk according to guideline directed calculators
  • Specialist cardiology input to assess cardiovascular risk – such as for patients with high cholesterol, high blood pressure or diabetes
  • Patients with a family history of heart disease

This program is not a screening program for the general public and is not suitable for individuals who are experiencing symptoms, or have already been diagnosed with heart disease.

Your Journey

How the pathway works:

1
Referral for appropriate patients

Identify patients at elevated cardiovascular risk through your exiting program.

2
Referral to Advara HeartCare (if appropriate)

For eligible patients, provide a valid referral for patients (online booking available) for access to HeartPath testing.

4
Online cardiology consult

A cardiologist reviews the individuals history, risk factors and findings via an online appointment.

5
Follow-up online consult for review and recommendations

Results are reviewed with a cardiologist, and any next steps if required are discussed. If further management or in-person care is required, the cardiologist will discuss recommended next steps and options with the individual.

Do patients need a referral?
Yes. A valid referral from a GP, Specialist or Nurse Practitioner is required to access this program. This ensures the assessment is clinically appropriate. We will work with our partners to determine what this pathway looks like for your patients.

The program provides specialist cardiology assessment. Any diagnosis, investigations or management recommendations are based on individual clinical findings and specialist judgement.

Yes. Services delivered as part of the defined HeartPath assessment pathway, including initial and follow up online consult, and in-clinic preliminary testing are bulk-billed unless otherwise advised. Patients identified with heart disease will be provided with their options beyond this program in their follow-up online consultation.

Suitability is determined based on clinical factors identified prior to referral. Not all patients will be appropriate for this pathway.

Following assessment, outcomes and recommendations are provided to support ongoing care. Further investigation or management may be advised depending on individual findings.

Yes. The program is delivered through a national cardiology network, supporting access regardless of location.

If an individual has already been diagnosed with heart disease, this program is not suitable for them. If they are not already under the care of a cardiologist, they can make an appointment with one of our Cardiologists at one of our many locations across Australia here.

If you are not close by to one of our clinics, our cardiologist can provide you with the referrals to visit a third party provider to conduct these tests on your behalf. Please check though with the provider regarding any out-of-pocket costs as we cannot guarantee that these are bulk-billed.
